Privacy policy

This is a translation for convenience. In case of any discrepancy, the Polish version prevails.

1. Data controller

2. Contact form

When you send a message through the form, I process the data you provide: your name, email address, phone number (if given) and the content of the message. I do this solely to reply to your message and, where relevant, arrange a visit (Art. 6(1)(b) and (f) GDPR).

If you describe your health problems in the message, this is health data. I process it on the basis of your explicit consent, which you give in the form (Art. 9(2)(a) GDPR). You can withdraw your consent at any time by writing to the address above. Withdrawing consent does not affect the lawfulness of processing before the withdrawal.

I keep messages in my mailbox for as long as needed to reply and continue the correspondence. If you become a patient, the rules on medical records apply.

3. Booking visits

The “Book a visit” button leads to online booking on the website of the Jest Lepiej Physiotherapy and Health Centre (jestlepiej.pl). Data processing during booking is described in that website’s privacy policy.

4. Cookies and statistics

This website does not use analytics tools, advertising pixels or tracking cookies. The only cookies that may appear are technical WordPress cookies needed for the website to work. Your choice of light or dark mode is saved only in your own browser.

Links to YouTube, Facebook, LinkedIn, ZnanyLekarz.pl, Google Play, the App Store and Google Maps lead to external services. Once you click, those services’ privacy policies apply.

The map on the Contact page loads from Google Maps only after you click “Show map”. At that point Google (Google Ireland Limited) receives technical data such as your IP address and may set its own cookies in line with its privacy policy. Without a click, no data is sent to Google.

5. Data recipients

Data may be passed to companies that technically operate the website and email (the hosting and email provider), only to the extent necessary for their work. I do not sell data or transfer it outside the European Economic Area.

6. Your rights

You have the right to access your data, rectify it, erase it, restrict processing, data portability and the right to object. You also have the right to lodge a complaint with the President of the Personal Data Protection Office (Prezes Urzędu Ochrony Danych Osobowych), ul. Stawki 2, 00-193 Warsaw, Poland.

Providing data in the form is voluntary, but without your name, email address and message I cannot reply.

7. Changes to this policy

I will announce any changes to this policy on this page. Version dated: website publication date.
